What orthopedic
therapy is for.

Pain is information, not a verdict. Orthopedic physical therapy is the work of finding out where yours is coming from, whether that is a joint, a muscle, a nerve, scar tissue or the way you have quietly learned to move around it, and then treating the cause rather than chasing the symptom.

Most of it is movement, load and strength, taught and progressed deliberately. Rest alone rarely fixes anything, and neither does pushing through. The useful ground is in between, and that is what gets mapped out with you.

Sometimes the pelvis is involved

Low back, hip and tailbone pain can turn out to involve the pelvic floor, and it often goes unlooked-at for years because most clinicians do not assess it. Here it is on the table from the start. If we find it is part of your picture we will explain why, and you decide whether you want to look at it.

  • Your history and your goal. What happened, what has been tried, and what you want to be doing again.
  • A movement assessment. How the painful area moves, and how everything around it is compensating.
  • Strength and control work, prescribed specifically rather than handed over as a generic sheet.
  • Load management, so you know what to do on a bad week as well as a good one.
  • Practical changes to how you sit, lift, sleep and work.
  • An explanation of what is driving your pain, in language you can repeat to someone else.
  • Progressions and regressions, reviewed as you go, so the plan tracks your body rather than a calendar.

Orthopedic care is largely assessment, teaching and progression, and that transfers well to a virtual appointment. You get coached through the exercises in your own space, using what you actually have, which tends to mean you keep doing them.

  • Works well virtually: history and goal setting, movement and strength assessment, exercise prescription and progression, pain education, load and activity planning, return to sport or lifting, workstation and daily habit changes.
  • Needs in person: hands-on manual therapy and joint mobilisation.

In-person visits are available to a limited number of patients, and depend on where you are and on whether hands-on treatment is actually the right thing for what is going on. Ask on your first contact and we will tell you honestly.

Do I need a scan or a diagnosis first?

Usually not. A good assessment tells us a great deal, and imaging often shows changes that are normal for your age and unrelated to your pain. If we think a scan or a medical opinion would genuinely change your plan, we will say so.

Will it hurt?

Treatment should not flare you badly. Some exercise produces mild, settling discomfort, and there is a real difference between that and pain that sets you back. You will be shown how to tell them apart, and you can stop at any point.

Can physical therapy really work without hands-on treatment?

For most orthopedic problems, yes. Manual therapy can feel good and can help in the short term, but strength, load and movement are what change the picture over time, and those are the parts you do yourself.

I have been told it is just wear and tear. Is it worth trying?

Almost always. Wear and tear on a scan is common in people with no pain at all. What tends to be changeable is strength, movement and how the area is being loaded.

What about fees, insurance and referrals?
